To report child access issues, where my partner / husband / wife has taken my child out of the country without my consent.
To report a concern for someone's welfare and / or safety.
Harassment is a course of conduct directed at, or towards an individual by another, that causes a victim to fear violence may be used against them or another person, or causes the victim to feel afraid, alarmed or distressed.
To report a hate crime or incident. Hate crime is any criminal offence which is motivated by prejudice or hostility (or perceived by the victim to be so) against a person’s race, religion, disability, sexual orientation or gender identity. A hate incident is something that may be experienced that is upsetting or frightening, which doesn't meet the threshold to be classed as a criminal offence.
To report modern slavery, which involves the movement or recruitment of individuals using threats, deception or other means for the purpose of exploitation.
To report suspicious activity that isn't covered in other reporting forms.
To be completed as fully as possible, by NHS staff only. The referral should be used to inform South Yorkshire Police, by a health care establishment, that a person has left care and is at high risk of coming to significant personal harm, or of causing such harm to another.
